Skilled nursing services from Private Duty Home Health Care are designed for clients who need medical care in the comfort of their own homes. Our services are delivered by highly trained Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) who provide a range of clinical support, including wound care, chronic disease management, medication administration, and post-surgical recovery. We start with an in-depth health assessment and collaborate with physicians to build a personalized care plan that meets your specific health needs while reducing the need for hospital visits.

These are clinical services ordered by a physician and provided by licensed nurses:

• Initial / ongoing nursing assessments
• Vital signs monitoring and symptom management
• Medication management, teaching, and reconciliation
• Medication administration (when allowed/ordered)
• Injections (IM/SubQ), insulin teaching, anticoagulant teaching
• Wound care (simple to complex)
• Surgical wound care, staples/suture removal (if ordered)
• Ostomy care and teaching
• Foley catheter insertion/change/care
• Straight catheterization (as ordered)
• Tracheostomy care and suctioning
• Enteral feeding care (PEG/NG tube) and education
• IV therapy (depending on state rules/agency capability)
• PICC line care and dressing changes
• Lab specimen collection (blood draws, urine samples)
• Chronic disease management (CHF, COPD, Diabetes, HTN)
• Infection prevention and monitoring
• Pain management and patient education
• Patient/caregiver teaching for disease process and safety
• Care coordination with physicians, case managers, and therapists
